I, on behalf of myself/my child, hereby acknowledge and agree to the terms set forth in this waiver ("Waiver") in consideration of being allowed to participate in sports and related activities organized by Youth Club of America, Inc. (YCA) and Global Orphan Aid for Life Inc. (GOAL).

Assumption of Risk: I understand and acknowledge that participation in sports activities, including but not limited to karate, martial arts, MMA, boxing, fitness workshops, basketball, soccer, swimming, surfing, rock climbing, and others, involves inherent risks of physical injury, including serious injury or death. I voluntarily assume all risks associated with participation in these activities.

Health and Medical Condition: I hereby certify that I am physically fit and have no known medical condition that would prevent my participation/my child's participation in the activities. I agree to inform the YCA and GOAL staff of any medical conditions or limitations that may affect my participation/my child's participation.

Release and Indemnity: I hereby release, waive, discharge, and covenant not to sue Youth Club of America, Inc., Global Orphan Aid for Life Inc. (GOAL), their officers, directors, employees, volunteers, agents, and representatives (collectively, "YCA and GOAL") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me/my child or to any property belonging to me/my child, whether caused by the negligence of YCA, GOAL, or otherwise, while participating in the sports activities.

Parental Consent for Minors: I am the parent or legal guardian of the minor named below. I hereby give my consent for the minor to participate in the sports activities organized by YCA and GOAL, and I agree to be bound by the terms of this Waiver on behalf of the minor.

Photographs: I understand that photographs and videos may be taken during YCA and GOAL sports activities and events. I hereby grant YCA and GOAL the irrevocable right and permission to use, publish, and display any photographs or recordings of me/my child for promotional, educational, or other purposes in any medium, including but not limited to online and social media platforms, without compensation or further notice.

Voluntary Participation: I acknowledge that my participation/my child's participation in YCA and GOAL sports activities is voluntary and that I have read and understand the terms of this Waiver. I agree to abide by all rules, regulations, and instructions provided by YCA and GOAL staff and coaches during the activities.

Severability: If any provision of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have read and understand this Waiver, and I voluntarily agree to be bound by its terms.
