Volunteer Waiver Agreement

Acknowledgment: I, on behalf of myself or my child, acknowledge and agree to the terms outlined in this waiver (“Waiver”) as a condition of participating in sports activities and volunteering at events such as food drives, charity events, fundraisers, or fairs organized by Youth Club of America, Inc. (YCA) and Global Orphan Aid for Life Inc. (GOAL).

Assumption of Risk: I understand that participating in sports activities and volunteering at events like food drives, charity events, fundraisers, or fairs involves inherent risks, including the risk of physical injury, serious injury, or death. I voluntarily accept all risks associated with these activities.

Health and Medical Condition: I certify that I am/my child is physically fit and have no known medical conditions that would prevent participation in these activities. I agree to inform YCA and GOAL staff of any medical conditions or limitations that may affect my/my child’s participation.

Release and Indemnity: I release, waive, discharge, and agree not to sue Youth Club of America, Inc., Global Orphan Aid for Life Inc. (GOAL), their officers, directors, employees, volunteers, agents, and representatives (collectively, “YCA and GOAL”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may occur to me/my child or my/my child’s property while participating in sports activities or volunteering at events, whether caused by the negligence of YCA, GOAL, or otherwise.

Parental Consent for Minors: I am the parent or legal guardian of the minor named below, and I consent to their participation in sports activities and volunteering at events organized by YCA and GOAL. I agree to be bound by the terms of this Waiver on behalf of the minor. 

Photographs: I understand that photographs and videos may be taken during YCA and GOAL activities and events. I grant YCA and GOAL the irrevocable right to use, publish, and display any photographs or recordings of me/my child for promotional, educational, or other purposes in any medium, including online and social media, without compensation or further notice.

Voluntary Participation: I acknowledge that my/my child’s participation in YCA and GOAL sports activities and event volunteering is voluntary. I have read and understand the terms of this Waiver and agree to follow all rules, regulations, and instructions provided by YCA and GOAL staff during these activities.

Severability: If any part of this Waiver is found to be invalid or unenforceable, the remaining provisions will remain in full effect.

Acknowledgment and Agreement: By signing below, I confirm that I have read and understand this Waiver, and I voluntarily agree to its terms.
